Initial Card State

  1.  All permanents on the battlefield (Leyline of Transformation with "Lhurgoyf" chosen).

Steps

  1. Activate Altar of Dementia by sacrificing Dark Revenant.
  2. When Dark Revenant dies, it triggers, putting itself from your graveyard onto the top of your library.
  3. Resolve the Altar of Dementia ability, causing you to mill cards equal to Dark Revenant's power, which will cause you to mill Dark Revenant.
  4. As Dark Revenant is a Lhurgoyf due to Leyline of Transformation, Disa's first ability triggers, returning Dark Revenant from your graveyard to the battlefield.
  5. Repeat.
