Initial Card State

  1.  Ulamog in hand.
  2.  All other permanents on the battlefield (Deathrender and Heirloom Blade attached to Ulamog).

Notable Prerequisites

  1. There are no other Eldrazi in your library unless those Eldrazi can also shuffle or put themselves into your library upon death.

Steps

  1. Activate Phyrexian Altar by sacrificing Ulamog, adding ({1} magic symbol)  .
  2. When Ulamog dies, it, Heirloom Blade and Deathrender trigger.
  3. Resolve the Ulamog trigger, shuffling it into your library.
  4. Resolve the Heirloom Blade trigger, causing you to reveal cards from the top of your library until you reveal Ulamog and put it into your hand.
  5. Resolve the Deathrender trigger, putting Ulamog from your hand onto the battlefield and attaching Deathrender to it.
  6. Activate Heirloom Blade's equip ability by paying ({1} magic symbol)  , attaching it to Ulamog.
  7. Repeat.
