Easy Prerequisites

  1. Kaya's Ghostform attached to Muldrotha.

Steps

  1. Activate Phyrexian Altar by sacrificing Muldrotha, adding one black mana.
  2. When Muldrotha dies, Kaya's Ghostform triggers, returning Muldrotha from your graveyard to the battlefield.
  3. Cast Kaya's Ghostform from your graveyard by paying one black mana, attaching it to Muldrotha.
  4. Repeat.

  1. Infinite death triggers.
  2. Infinite creature ETB.
  3. Infinite creature LTB.
  4. Infinite creature sacrifice triggers.
